Job Overview
Provide comprehensive medication management services in the Advanced Therapies clinic through active participation in M Health Fairview Genetics and Metabolism clinic appointments and consistent follow-up with clinic patients. Work directly with providers, nurses, dieticians, and other health care professionals to triage medication therapy problems. Facilitate therapy plan initiation and renewal with associated lab orders and referrals under the collaborative practice agreement. Identify and resolve medication therapy problems and accept accountability to achieve established goals of therapy. Documentation within local electronic medical record (EMR) as indicated.
Responsibilities
- Provide comprehensive medication management services to patients in the Advanced Therapies clinic.
- Act under a collaborative practice agreement with assigned providers
- Ensure all drugs are appropriately indicated, effective, safe, and convenient for the patient.
- Identify, resolve, and prevent medication therapy problems.
- Perform initial patient evaluation as it pertains to the prescribed therapies. Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy. Obtain histories and apply these along with other patient data to optimize drug therapy management. Work with other healthcare providers to evaluate patients to determine appropriateness for oral and injectable specialty medications along with home and infusion center IV therapy.
- Evaluate the signs and symptoms relative to disease processes and make recommendations and/or modifications of medication therapy
- Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy. Adjust administration of medication and consult with patient’s provider if orders need to be modified for safe medication administration.
- Ensure that the patient understands the specific goals of therapy and that the patient and providers are working to achieve these goals.
- Lead the pegvaliase monitoring program to adjust dosing, frequency, and associated pre-medications to meet the individualized needs of each patient.
- Manage the incoming phone calls and faxes to the clinic phone from patients, payers, pharmacies, etc.
- Maintain relationships with manufacturers and pharmaceutical representatives to drive optimal patient outcomes. Attend reoccurring monthly/bimonthly meetings and communicate promptly through email/phone for any pressing patient case matters.
- Be readily available for on demand submission of prior authorizations, letters of medical necessity, and appeal letters.
- Coordinate with outside pharmacies and home infusion agencies as the primary contact for the Advanced Therapies clinic to ensure continued access to medications, supplies, and resources for patients on a prescribed therapy.
- Maintain proficiency in the skills and theory of the practice of pharmacy, with emphasis on phenylketonuria, lysosomal storage disorders, and other genetic/metabolic disorders, by reviewing literature and attending in-services, seminars, and conferences.
- Act as the primary preceptor for any learners (students and residents) on rotations with the Advanced Therapies Pharmacist
- Engage in research through abstract submissions and authoring papers in clinical areas of expertise
Requirements
Required Qualifications
- Bachelor of Science Pharmacy or PharmD from an accredited school of pharmacy
- Pharmacist MN Board of Pharm
Preferred Qualifications
- Prior work experience in specialty pharmacy and/or home infusion pharmacy services. Direct clinical management with specialty populations, preferably with rare diseases Residency with an ambulatory focus
- Certified Specialty Pharmacist (CSP)
