Description
Piedmont Healthcare is seeking a dedicated Pharmacist to join our Healthcare & Medical Services team. In this role, you will verify and dispense medications, provide clinical guidance, and collaborate with physicians, nurses, and other clinicians to ensure safe, effective, and patient-centered care. You will monitor medication therapies, counsel patients, and support quality and safety initiatives. Conduct clinical medication assessments, including dose adjustments, renal and hepatic dosing, and therapeutic drug monitoring
Identify and resolve drug interactions, allergies, duplications, and contraindications to promote safe medication use
Participate in medication reconciliation to ensure accurate medication histories and transitions of care
Educate patients, families, and healthcare staff on medication indications, side effects, and adherence strategies
Document clinical interventions and pharmacist recommendations in the electronic health record and pharmacy systems
Support and participate in antimicrobial stewardship, medication safety, and other quality improvement initiatives
Collaborate with physicians, nurses, and other healthcare professionals during multidisciplinary rounds and care planning
Assist with formulary management and adherence to institutional protocols and evidence-based guidelines
Work variable shifts, including evenings, nights, weekends, and holidays as needed, to ensure continuous pharmacy services
Clinical pharmacotherapy knowledge across common inpatient and outpatient disease states
Proficiency with electronic health records (EHR) and pharmacy information systems
Medication order verification and dispensing accuracy
Therapeutic drug monitoring and dose adjustment skills
Knowledge of drug interactions, contraindications, and adverse effect profiles
Familiarity with antimicrobial stewardship principles and medication safety practices
Strong written and verbal communication with patients and interprofessional teams
Patient counseling and education on medication use and adherence
Time management and ability to prioritize in a fast-paced clinical environment
Understanding of regulatory standards and pharmacy law in a hospital/health-system setting

Active and unrestricted Pharmacist license in the state of practice
Doctor of Pharmacy (PharmD) or equivalent from an accredited school of pharmacy
Basic Life Support (BLS) certification or ability to obtain within specified timeframe
Immunization certification (e.g., pharmacist vaccine administration) or willingness to obtain
Experience with electronic health records (EHR) and computerized physician order entry (CPOE) systems
