Description

Encompass Health seeks a Pharmacist to support our inpatient rehabilitation hospital within Healthcare & Medical Services. In this role, you will ensure safe, accurate medication dispensing, optimize medication therapy, and provide patient and caregiver counseling. You will collaborate with physicians, nurses, and the interdisciplinary rehab team to monitor drug interactions, manage complex regimens, and support recovery goals. Dispense prescribed medications accurately and safely
Review and verify medication orders for appropriateness and safety
Provide medication counseling and education to patients and caregivers
Collaborate with physicians, nurses, and rehab team on medication therapies
Monitor for drug interactions, side effects, and therapeutic outcomes
Maintain accurate pharmacy records and comply with regulatory standards
Oversee medication storage, handling, and inventory control
Participate in quality improvement and patient safety initiatives
Pharmacology knowledge
Medication therapy management (MTM)
