Cardinal Hill Rehabilitation Hospital, part of Encompass Health, is seeking a Pharmacist to join our Healthcare & Medical Services team. In this role, you will ensure safe, effective medication therapy, collaborate closely with physicians, nurses, and therapists, and provide patient education on medication use. You’ll verify orders, manage drug distribution, monitor for interactions, and support quality and regulatory standards. Review, verify, and dispense medications in accordance with physician orders, hospital policies, and regulatory standards
Provide clinical pharmacotherapy oversight, including medication therapy management and optimization for rehab and medically complex patients
Conduct thorough medication reconciliation at admission, transfer, and discharge to ensure safe transitions of care
Educate patients and families on medication regimens, indications, side effects, and adherence strategies tailored to rehabilitation goals
Collaborate closely with physicians, nurses, therapists, and case managers to support evidence-based, patient-centered care plans
Monitor for and help prevent adverse drug events, drug interactions, and contraindications, and provide timely clinical recommendations
Participate in multidisciplinary rounds and contribute pharmacologic expertise to treatment planning and discharge readiness
Support safe and compliant handling of controlled substances, including inventory control, documentation, and auditing
Ensure compliance with all applicable pharmacy laws, Joint Commission, CMS, and hospital policies and procedures
Assist with formulary management, therapeutic interchanges, and cost-effective medication use while maintaining quality outcomes
Document all clinical interventions, counseling, and medication-related activities accurately in the electronic medical record
Participate in quality improvement initiatives, medication use evaluations, and performance metrics to enhance patient outcomes
Provide guidance and support to pharmacy technicians and other team members, fostering a culture of safety and collaboration
Contribute to staff education on medication updates, protocols, and best practices in rehabilitation and acute care pharmacy

Active, unrestricted Doctor of Pharmacy (PharmD) degree from an ACPE-accredited program
Current pharmacist licensure in the state of practice, in good standing
Hospital or clinical pharmacy experience, preferably in rehabilitation, acute care, or similar setting
Completion of a PGY1 pharmacy residency preferred but not required
Board certification (e.g., BCPS or other relevant BPS credential) preferred but not required
Immunization certification and authorization to administer vaccines, if required by state regulations
