Description
Cardinal Hill Rehabilitation Hospital, part of Encompass Health, is seeking a Pharmacist to join our Healthcare & Medical Services team. In this role, you will ensure safe, effective medication therapy, collaborate closely with physicians, nurses, and therapists, and provide patient education on medication use. You’ll verify orders, manage drug distribution, monitor for interactions, and support quality and regulatory standards. Provide clinical pharmacy services including medication therapy management, drug information, and patient counseling to optimize outcomes
Collaborate with physicians, nurses, and the interdisciplinary rehab team to develop and adjust individualized medication plans
Perform medication reconciliation on admission, transfer, and discharge to ensure accuracy and continuity of care
Monitor for and help prevent adverse drug events, drug interactions, and contraindications, documenting and reporting as required
Support rehabilitation goals through evidence-based recommendations tailored to complex, multi-morbid patient populations
Maintain accurate records in the electronic health record and pharmacy systems, ensuring timely and clear documentation
Participate in quality improvement initiatives, antimicrobial stewardship, and medication use evaluations
Oversee and support pharmacy technicians in medication preparation, distribution, and inventory processes
Ensure compliance with all federal and state pharmacy regulations, Joint Commission standards, and hospital policies
Assist in managing formulary, medication inventory, and controlled substances, including proper storage, security, and documentation
Provide education to patients, families, and staff on safe and effective medication use, side effects, and adherence strategies

Active, unrestricted Doctor of Pharmacy (PharmD) degree from an ACPE-accredited program
Current state pharmacist license in good standing in the state of practice
Completion of a PGY-1 Pharmacy Practice Residency in hospital or clinical pharmacy preferred
Board certification in Pharmacotherapy (BCPS) or other relevant specialty preferred but not required
Current Basic Life Support (BLS) certification; ACLS preferred depending on hospital policy
Experience with hospital pharmacy systems, EHRs, and automated dispensing technology preferred
