Description
Cardinal Hill Rehabilitation Hospital, part of Encompass Health, is seeking a Pharmacist to join our Healthcare & Medical Services team. In this role, you will ensure safe, effective medication therapy, collaborate closely with physicians, nurses, and therapists, and provide patient education on medication use. You’ll verify orders, manage drug distribution, monitor for interactions, and support quality and regulatory standards. Provide clinical pharmacy services including medication therapy management, pharmacokinetic dosing, and drug regimen review for rehabilitation patients.
Perform thorough medication reconciliation on admission, transfer, and discharge to ensure safety and continuity of care.
Educate patients, families, and caregivers on medication use, side effects, and adherence strategies tailored to rehabilitation needs.
Collaborate closely with physicians, nurses, therapists, and case managers as part of the interdisciplinary team to optimize therapeutic outcomes.
Monitor for and help prevent adverse drug events, drug interactions, and contraindications, documenting and reporting as required.
Support safe preparation and verification of IV and other high-risk medications, following established protocols and standards.
Participate in quality improvement initiatives, medication use evaluations, and implementation of evidence-based best practices.
Maintain accurate, timely documentation in the electronic health record and support compliance with TJC, CMS, and state board of pharmacy regulations.
Contribute to formulary management, inventory oversight, and cost-effective medication use within the rehabilitation hospital.
Provide clinical guidance and drug information to healthcare team members, staying current with emerging therapies and best practices.
Precept or mentor new staff or students as appropriate, fostering a culture of learning, collaboration, and continuous improvement.

Pharmacokinetics and dosing adjustment

Reviewing and interpreting physician orders
Clinical decision support systems (CDSS) use

Active, unrestricted PharmD or BS Pharmacy license in the state of employment
Completion of an ASHP-accredited PGY1 Pharmacy Residency preferred or equivalent clinical experience
Basic Life Support (BLS) certification, or ability to obtain within designated timeframe
Advanced Cardiac Life Support (ACLS) preferred, depending on patient population
Immunization certification where applicable by state law
Previous hospital, rehabilitation, or acute care pharmacy experience preferred
