Description
Cardinal Hill Rehabilitation Hospital, part of Encompass Health, is seeking a Pharmacist to join our Healthcare & Medical Services team. In this role, you will ensure safe, effective medication therapy, collaborate closely with physicians, nurses, and therapists, and provide patient education on medication use. You’ll verify orders, manage drug distribution, monitor for interactions, and support quality and regulatory standards. Collaborate closely with physicians, nurses, and the interdisciplinary rehab team to optimize medication therapy and support individualized care plans.
Conduct comprehensive medication reconciliation and ongoing medication therapy management to improve outcomes and minimize adverse events.
Provide patient and family education on medication indications, dosing, side effects, interactions, and safe administration techniques.
Participate in clinical initiatives such as antimicrobial stewardship, pain management optimization, and chronic disease medication management.
Monitor for and document adverse drug reactions, medication errors, and near-misses, contributing to quality and safety improvement efforts.
Ensure compliance with federal, state, and institutional regulations, including controlled substance management and documentation standards.
Prepare and verify sterile and non-sterile compounded medications in accordance with USP and hospital policies when needed.
Support formulary management by evaluating medication requests, providing drug information, and helping develop evidence-based protocols and order sets.
Use electronic health records and pharmacy information systems to manage orders, document interventions, and communicate with the care team.
Contribute to staff education and in-services for nursing and other clinical disciplines on safe and effective medication use.
Participate in performance improvement projects and hospital committees that advance medication safety, clinical outcomes, and rehab-focused care.
Help orient, mentor, and precept pharmacy students or new team members, modeling Encompass Health Cardinal Hill’s culture of integrity and collaboration.
Clinical pharmacy practice in an acute inpatient or rehabilitation setting
Medication therapy management and medication reconciliation
Pharmacokinetics and dose adjustment for renal/hepatic impairment
Sterile and non-sterile compounding standards (USP <797>/<800>)
Regulatory compliance (Joint Commission, CMS, state board of pharmacy)
Use of electronic health records (EHR) and pharmacy information systems
Verification and dispensing of medications, including high-alert drugs
IV admixture and TPN preparation/verification
Interdisciplinary care planning and clinical rounding
Patient and family education on medication use and safety
Drug information research and formulary management
Adverse drug event monitoring and reporting
Antimicrobial stewardship principles
Inventory control, including controlled substances management
Precepting and mentoring pharmacy students or residents

Active Doctor of Pharmacy (PharmD) degree from an ACPE-accredited program
Current, unrestricted pharmacist license in the state of practice
Hospital or health-system pharmacy experience (clinical rotations or employment)
Completion of a PGY1 Pharmacy Practice Residency preferred but not required
BLS (Basic Life Support) certification required or obtainable within specified timeframe
Advanced life support certification (e.g., ACLS) preferred for acute/rehab hospital practice
Board certification (e.g., BCPS) preferred or interest in obtaining is a plus
