Description
Cardinal Hill Rehabilitation Hospital, part of Encompass Health, is seeking a Pharmacist to join our Healthcare & Medical Services team. In this role, you will ensure safe, effective medication therapy, collaborate closely with physicians, nurses, and therapists, and provide patient education on medication use. You’ll verify orders, manage drug distribution, monitor for interactions, and support quality and regulatory standards. Collaborate with physicians, nurses, and the interdisciplinary rehabilitation team to optimize pharmacotherapy and support individualized care plans.
Provide clinical pharmacokinetic dosing, monitoring, and adjustment for select medications to ensure efficacy and safety.
Conduct medication therapy management, drug interaction screening, and drug utilization review to minimize adverse events and improve outcomes.
Counsel patients and caregivers on medication use, side effects, adherence strategies, and safe storage, tailored to the rehabilitation population.
Support safe transitions of care by performing medication reconciliation at admission, transfer, and discharge.
Prepare and oversee sterile and non-sterile compounding, including IV admixtures, using appropriate aseptic technique and safety standards.
Maintain accurate clinical documentation in the electronic health record and ensure completeness of medication profiles.
Monitor, manage, and optimize pharmacy inventory, including controlled substances, to support continuous patient care and regulatory compliance.
Ensure compliance with hospital, state, and federal regulations, as well as Joint Commission and CMS standards related to pharmacy practice.
Participate in quality improvement initiatives, medication safety projects, and clinical protocol development to enhance patient outcomes.
Provide education to clinical staff on new medications, protocols, and safe medication practices.
Assist in orientation and mentoring of new pharmacy staff, students, or residents as applicable.

Completion of ASHP-accredited PGY1 residency preferred but not required
Advanced certifications such as BPS board certification (e.g., BCPS) preferred but not required
